Product Description:

Tramadol works in two ways at once. It is a weak mu opioid agonist — its affinity is a small fraction of morphine's — and it also inhibits reuptake of noradrenaline and serotonin, strengthening the descending pathways that suppress pain. Much of its opioid effect comes from the metabolite O-desmethyltramadol, produced by CYP2D6, which is far more potent at the receptor than the parent drug.

It is used for moderate to severe pain, and the injection is used after surgery and in acute pain where oral dosing is not possible. That CYP2D6 dependence makes response unpredictable: poor metabolisers get little opioid effect, while ultra-rapid metabolisers can reach dangerous concentrations, which is why it is not used in children or in breastfeeding. Seizures occur at higher doses and in patients on antidepressants, and serotonin syndrome can develop when it is combined with SSRIs, SNRIs, MAO inhibitors or triptans. Dependence develops, and abuse has become a significant problem in several regions.

Tramadol is not listed under the international drug conventions, but it is nationally controlled in many countries — including much of the Middle East and North Africa, where import is tightly restricted or prohibited.
