Product Description:

Dinoprostone is prostaglandin E2, identical to the compound the body produces. In the cervix it activates collagenase and alters the extracellular matrix, softening and shortening it — ripening — and it also stimulates uterine muscle to contract. Both actions are needed before labour can progress, which is why it is the standard agent for induction when the cervix is unfavourable.

It is used for cervical ripening and induction of labour at or near term, and in some markets for second-trimester termination and for management of missed miscarriage. The vaginal insert releases drug slowly and, crucially, can be removed if the uterus over-responds; the gel and tablet cannot. Uterine hyperstimulation with fetal heart rate changes is the main risk, so administration is in hospital with continuous monitoring. It is contraindicated where vaginal delivery is not appropriate, and used with great caution after previous caesarean section because of uterine rupture risk.

Sourcing notes. Cold chain is the defining logistic: the vaginal insert and gel require frozen or refrigerated storage depending on presentation, and potency is lost if the chain breaks. Hospital and maternity-unit purchasing only. The insert, gel and tablet are separate registrations with different handling.
