Cefiderocol is a siderophore cephalosporin, and its mechanism is genuinely novel. Gram-negative bacteria actively scavenge iron from their environment using siderophores. Cefiderocol chelates ferric iron and is then transported into the periplasmic space through the bacterium's own iron uptake channels — a "Trojan horse" route that bypasses the porin loss and efflux pumps by which resistant organisms exclude conventional beta-lactams.
The result is activity against carbapenem-resistant Enterobacterales, Pseudomonas aeruginosa, Acinetobacter baumannii and Stenotrophomonas maltophilia, including metallo-beta-lactamase producers against which most other agents fail. It is indicated for complicated urinary tract infection and for hospital-acquired and ventilator-associated pneumonia caused by susceptible Gram-negative organisms, supplied as 1 g powder for injection.
