Reference Brands: Fetroja (USA), Fetcroja (EU/UK)

Category: Antibiotics

Cefiderocol is a siderophore cephalosporin antibiotic used to treat serious Gram-negative bacterial infections, including those caused by multidrug-resistant organisms. It works by binding to extracellular iron, using bacterial iron uptake systems to penetrate and inhibit cell wall synthesis. It is particularly effective against carbapenem-resistant strains such as Pseudomonas aeruginosa and Acinetobacter baumannii. Cefiderocol is available as a 1 g IV powder under the brand names Fetroja and Fetcroja.

Product Description:

Cefiderocol is a siderophore cephalosporin, and its mechanism is genuinely novel. Gram-negative bacteria actively scavenge iron from their environment using siderophores. Cefiderocol chelates ferric iron and is then transported into the periplasmic space through the bacterium's own iron uptake channels — a "Trojan horse" route that bypasses the porin loss and efflux pumps by which resistant organisms exclude conventional beta-lactams.

The result is activity against carbapenem-resistant Enterobacterales, Pseudomonas aeruginosa, Acinetobacter baumannii and Stenotrophomonas maltophilia, including metallo-beta-lactamase producers against which most other agents fail. It is indicated for complicated urinary tract infection and for hospital-acquired and ventilator-associated pneumonia caused by susceptible Gram-negative organisms, supplied as 1 g powder for injection.

Sourcing notes, and these are decisive. Cefiderocol is a patented innovator product from a single manufacturer. No generic has been approved anywhere, the underlying patents were filed in 2019, and realistic generic entry is more than a decade away. It cannot be sourced on a bulk or contract manufacturing basis, and an offer of generic cefiderocol should be refused and investigated. WHO classifies it under Reserve, the most restricted AWaRe category, meaning its use is confined to confirmed multidrug-resistant infection under stewardship control. Legitimate supply runs through authorised distribution, or through named-patient import where the product is not registered locally.

How Cefiderocol is actually supplied

Named-patient import Where a patient needs the product in a market where it is not registered, named-patient import is the lawful route. It requires the treating physician's documentation and authorisation from the destination regulator. We handle this pathway. Named-patient access
Authorised distribution For registered markets, supply runs through the manufacturer's authorised distribution channel. We can identify the authorised route for your market and tell you whether the product is registered there at all. Innovator sourcing
A genericised alternative Where the clinical requirement is the drug class rather than this specific molecule, there is often an off-patent alternative that is faster to register and materially cheaper. Tell us the indication and target market and we will say plainly whether one exists. Discuss alternatives

It is indicated for the treatment of complicated urinary tract infections, hospital-acquired bacterial pneumonia, ventilator-associated bacterial pneumonia, and infections due to carbapenem-resistant pathogens when no suitable alternatives are available.

Cefiderocol is made from the active ingredient cefiderocol, a siderophore cephalosporin antibiotic. It works by binding to iron and using bacterial iron transport systems to enter bacterial cells, where it inhibits cell wall synthesis, leading to bacterial death. Its chemical name is (6R,7R)-7-[[(2Z)-2-(2-aminothiazol-4-yl)-2-methoxyiminoacetyl]amino]-3-[2-(2,3-dihydroxybenzoyl)oxypropyl]-8-oxo-5-thia-1-azabicyclo[4.2.0]oct-2-ene-2-carboxylic acid.

The trade name of cefiderocol is Fetroja in the United States and Fetcroja in the European Union and United Kingdom.

Cefiderocol (Fetroja/Fetcroja) is manufactured and marketed by Shionogi & Co., Ltd., a Japanese pharmaceutical company.

The generic name of the drug is cefiderocol.

The brand names of cefiderocol are Fetroja (USA) and Fetcroja (EU/UK).
