Demeclocycline is a semisynthetic tetracycline antibiotic used to treat a variety of bacterial infections including respiratory, urinary, skin, eye, and tick-borne illnesses. It also has a unique “off-label” role in treating the syndrome of inappropriate antidiuretic hormone secretion (SIADH) by reducing renal sensitivity to ADH, thereby promoting water excretion. It must be taken on an empty stomach (at least 1 hour before or 2 hours after meals, especially avoiding dairy products) due to absorption issues. Common adverse effects include photosensitivity, tooth discoloration in children, and potential for nephrogenic diabetes insipidus when used in high doses.

Demeclocycline is a tetracycline antibiotic, but in modern practice it is prescribed far more often for a side effect than for its antibacterial activity. It induces nephrogenic diabetes insipidus by antagonising the action of antidiuretic hormone on the renal collecting duct, and that effect is used deliberately to treat hyponatraemia caused by the syndrome of inappropriate ADH secretion.

As an antibacterial it shares the tetracycline spectrum and limitations. It is supplied as 150 and 300 mg tablets.

Two class effects matter. Demeclocycline causes marked photosensitivity, more so than other tetracyclines, and patients need explicit warning. And as a tetracycline it is contraindicated in pregnancy and in children under eight because of permanent tooth discoloration and effects on bone growth.

Sourcing notes. Because its principal use is off-label for SIADH, demand is specialised and modest, concentrated in hospital nephrology and endocrinology rather than general practice. Manufacturers and current registrations are correspondingly few, and it has been discontinued in some markets — confirm availability before committing to a tender. Where the requirement is genuinely hyponatraemia management, tolvaptan is the licensed alternative in many markets, though considerably more expensive.
