Cycloserine is a broad-spectrum antibiotic used primarily in the treatment of multidrug-resistant tuberculosis (MDR-TB). It acts as a cell-wall synthesis inhibitor, competing with D-alanine, an essential component of bacterial cell walls. This leads to inhibition of bacterial growth. Because of potential neurotoxicity, it is generally reserved for cases resistant to first-line TB drugs.

Cycloserine is a second-line antituberculosis agent, a structural analogue of D-alanine that inhibits bacterial cell wall synthesis by blocking the enzymes incorporating D-alanine into peptidoglycan. It is used exclusively within combination regimens for multidrug-resistant tuberculosis, never as monotherapy, and is supplied as 250 mg capsules.

Neuropsychiatric toxicity is the defining limitation. Cycloserine crosses the blood-brain barrier and produces a spectrum of central nervous system effects — headache, irritability, depression, psychosis and seizures. These are dose-related and more frequent in renal impairment and with concurrent alcohol use. Pyridoxine supplementation is routinely given to mitigate them, and psychiatric monitoring is part of the treatment protocol rather than an optional extra. Patients with existing psychiatric illness require particular caution.

Sourcing notes. Second-line TB medicines move principally through public health procurement — national TB programmes, the Global Drug Facility and donor-funded mechanisms — rather than commercial distribution. WHO prequalification or stringent regulatory authority approval is frequently an eligibility condition for these tenders, and it is a higher threshold than national GMP alone.
