Fidaxomicin is a macrolide antibiotic used to treat Clostridioides difficile–associated diarrhea (C. difficile infection). It works by inhibiting bacterial RNA polymerase, which stops bacterial growth in the gut while maintaining normal intestinal flora.

Fidaxomicin is a macrocyclic antibiotic used exclusively for Clostridioides difficile infection, and its value rests on what it does not do. It is minimally absorbed, remaining in the gut lumen where the infection is, and it has an unusually narrow spectrum that spares the anaerobic gut flora. Because that flora is what normally resists C. difficile colonisation, preserving it substantially reduces recurrence — the principal problem in this infection, where a quarter or more of patients relapse after treatment with vancomycin or metronidazole.

It is supplied as 200 mg tablets and as oral suspension at 40 mg per mL for paediatric use.
