Aztreonam is a monobactam antibiotic used to treat a wide range of Gram-negative bacterial infections. It works by inhibiting bacterial cell wall synthesis, leading to cell death. Unlike other β-lactam antibiotics, it has minimal cross-reactivity with penicillins or cephalosporins, making it a safe alternative for patients with penicillin allergy.

Aztreonam is the only monobactam in clinical use, and its structural distinctiveness gives it a property no other beta-lactam has: minimal cross-reactivity with penicillins and cephalosporins in patients with severe beta-lactam allergy. For a patient with documented anaphylaxis to penicillin who needs Gram-negative cover, aztreonam is frequently the answer, and that alone sustains its place in formularies.

Its spectrum is exclusively aerobic Gram-negative, including Pseudomonas aeruginosa. It has no useful Gram-positive or anaerobic activity, so it is not a monotherapy for mixed infection.

Two presentations exist and they are quite different products. The intravenous powder at 1 g and 2 g treats systemic infection. The 75 mg inhalation solution is a preservative-free formulation for chronic Pseudomonas suppression in cystic fibrosis, delivered by a specific nebuliser system.

Sourcing notes. Do not treat the two as one product. The inhalation formulation is device-linked, uses a lysine salt rather than the arginine formulation, and has its own registration. WHO classifies aztreonam under Reserve, which restricts its use in stewardship programmes and shapes tender eligibility. Sterile manufacture is required for both, with the inhalation product demanding a narrower approval scope.
