Cefotetan is a broad-spectrum cephalosporin antibiotic that works by inhibiting bacterial cell wall synthesis. It is especially effective against anaerobic bacteria and certain Gram-negative organisms. Commonly used for surgical infection prevention and treatment of gynecological, urinary, and intra-abdominal infections.

Product Description:

Cefotetan is a cephamycin, grouped with the second-generation cephalosporins but distinguished by substantial activity against anaerobes, including Bacteroides fragilis. That anaerobic cover is the reason it persists in use: it allows single-agent prophylaxis and treatment in colorectal, gynaecological and other procedures where mixed aerobic-anaerobic flora is expected.

It is supplied as powder for injection at 1 g and 2 g, with a long half-life permitting twice-daily dosing.

Two cautions belong in the labelling. Cefotetan contains an N-methylthiotetrazole side chain, which is associated with hypoprothrombinaemia and bleeding — vitamin K monitoring or supplementation is advised in prolonged treatment, in malnourished patients and in renal impairment. The same side chain produces a disulfiram-like reaction with alcohol, which patients must be warned about explicitly.

Sourcing notes. Cefotetan has narrowed in availability as combination regimens displaced it, and it has been discontinued in several markets, so current manufacturers and registrations are fewer than the class would suggest. Where a buyer specifies it, confirm supply before committing to a tender. Manufacture requires sterile powder-filling approval at the specific plant.
