Gatifloxacin is a fourth-generation fluoroquinolone antibiotic used to treat a variety of bacterial infections. It works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication and repair. The ophthalmic formulations are widely used for bacterial conjunctivitis, while the oral form has been withdrawn in several countries due to rare but serious blood sugar-related adverse effects.

Gatifloxacin is a fourth-generation fluoroquinolone, and its history is instructive. Systemic oral and injectable gatifloxacin was withdrawn from major markets after reports of severe dysglycaemia — both hypoglycaemia and hyperglycaemia, some fatal, particularly in elderly and diabetic patients. India banned the systemic formulations in 2011.

The ophthalmic formulation is a different proposition and remains widely used. Topical application to the eye produces negligible systemic absorption, so the dysglycaemia risk that ended the systemic product does not apply. Gatifloxacin eye drops at 0.3% and 0.5% are used for bacterial conjunctivitis and in perioperative ophthalmic prophylaxis, where good corneal penetration and broad activity are advantages.

Ophthalmic manufacture requires sterile approval specifically covering eye preparations, which is narrower than general sterile scope and must be confirmed at plant level.
