Imipenem/cilastatin, is an antibiotic useful for the treatment of a number of bacterial infections. It is made from a combination of imipenem and cilastatin. Specifically it is used for pneumonia, sepsis, endocarditis, joint infections, intra-abdominal infections, and urinary tract infections. It is given by injection into a vein or muscle.

Imipenem is a carbapenem with one of the broadest antibacterial spectra available — Gram-positive, Gram-negative including Pseudomonas aeruginosa, and anaerobes. Cilastatin is not an antibiotic. It inhibits renal dehydropeptidase-I, the enzyme that would otherwise degrade imipenem in the kidney, and its inclusion in a 1:1 ratio is what makes the drug usable at all.

It is used in severe hospital-acquired infection, intra-abdominal sepsis and febrile neutropenia, and is supplied as powder for injection at 500 mg and 1 g of each component.

Two practical points. Imipenem carries a greater seizure risk than other carbapenems, particularly in renal impairment, in central nervous system disease and at high doses — meropenem is generally preferred where CNS infection is suspected. And strength is quoted inconsistently: a "500 mg vial" may mean 500 mg imipenem with 500 mg cilastatin, or 500 mg total. Settle the basis in the purchase order.

Sourcing notes. WHO classifies carbapenems under Watch, and stewardship programmes restrict their use tightly. Manufacture requires sterile powder-filling approval at the specific plant. Hospital tender volumes are significant and generic supply is well established.
