Supplying Medicines and Supplements to South Africa
A practical guide for South African importers, wholesalers and hospital buyers who source from abroad, and for manufacturers and exporters worldwide selling into South Africa.
South Africa is the largest and most sophisticated pharmaceutical market in sub-Saharan Africa, with a strong regulator, SAHPRA, a regulated price system and central government tenders.
SAHPRA's 2025 reliance guideline makes life easier for products already approved by trusted regulators, and a site approved by a recognised regulator in the past three years may not need a SAHPRA inspection.

How to supply medicines to South Africa
- Appoint your local applicant. A foreign company applies through a South African responsible person, with a registered pharmacist signing the application.
- Prepare the dossier. A ZA-CTD dossier (eCTD preferred), with GMP evidence for the plant and a CoPP.
- Choose the route. Standard review (trade press reports targets of about 250 working days for generics) or reliance (about 200), if the product is identical to one assessed by a reference regulator.
- Supply. Private wholesalers and pharmacy groups at the Single Exit Price, or National Department of Health contracts, scored on price plus points for specific goals.
Section 21 authorisations let a doctor obtain an unregistered medicine for named patients through a licensed importer, with a stated turnaround of up to three working days. See named-patient and special import and government tender supply.
Supplements
Health supplements can fall under SAHPRA's complementary medicines rules or under food rules, depending on ingredients and claims. Confirm the classification first. We help brands get supplements made under their own label by GMP-certified contract manufacturers: see nutraceutical private label.
What people in South Africa search for
Semrush data from October 2026 shows that South Africans search most for ashwagandha (about 74,000 searches a month), magnesium glycinate, collagen, vitamin D, probiotics and B12. Among medicines, vitamin B, allergy and pain treatments lead.
South Africans search in English, and often by format: “ashwagandha pills” gets about 33,100 searches a month and “collagen powder” 18,100, alongside plain “magnesium” (22,200) and “vitamin c” (18,100) (Semrush, October 2026). For medicines they use generic names such as “prednisolone” (about 14,800 a month), “diclofenac” (14,290) and “folic acid” (13,100).
How importers and distributors in South Africa search
Most trade searches in South Africa name a specific wholesaler or distributor; the general phrase “private label” gets about 140 a month, and “private label food manufacturers south africa” about 30 (Semrush, October 2026).
Originator brands searched by name include Brufen (ibuprofen), Flagyl (metronidazole), Ozempic (semaglutide), Buscopan (hyoscine butylbromide), Voltaren (diclofenac) and Nurofen (ibuprofen). Brand names are trademarks of their owners and are used only to identify the medicine.

Among the most searched generic medicines are prednisone, tramadol, ivermectin, diclofenac, ibuprofen and folic acid.
