Ivermectin is a broad-spectrum antiparasitic medicine derived from the avermectins, natural products of the soil bacterium Streptomyces avermitilis. Its discovery earned William Campbell and Satoshi Ōmura a share of the 2015 Nobel Prize in Physiology or Medicine. In people it is used for strongyloidiasis and onchocerciasis (river blindness), in many countries for scabies and sometimes head lice, and in mass drug administration against lymphatic filariasis. The dose depends on body weight, usually 150 to 200 micrograms per kilogram as a single dose. The originator brand is Stromectol (MSD), supplied as 3 mg tablets; 6 mg and 12 mg tablets are common in India and other markets.

Who makes ivermectin?

Ivermectin is long off patent. The API is made by fermentation followed by chemical hydrogenation of avermectin B1 (abamectin), and is produced in large volumes in India and China for both human and veterinary use. Many Indian companies make ivermectin tablets, and India uses ivermectin together with diethylcarbamazine and albendazole (the IDA regimen) in high-burden districts of its programme to eliminate lymphatic filariasis.

Human versus veterinary ivermectin

Ivermectin is also one of the most widely used veterinary antiparasitics, in injections, pour-ons and oral products. Veterinary products and veterinary-grade API must not be used in human medicines. For human use, ask for API to the USP, Ph. Eur. or IP monograph from a site with a GMP certificate for human medicines, and tablets registered for human use in your market. Ivermectin is not approved for COVID-19 by the US FDA or the European Medicines Agency, and WHO and EMA advise against its use for COVID-19 outside clinical trials.
