Ganirelix acetate is a synthetic decapeptide and a gonadotropin-releasing hormone (GnRH) antagonist used to control ovarian stimulation in women undergoing assisted reproductive treatments. It works by directly blocking the GnRH receptors in the pituitary gland, resulting in rapid suppression of luteinizing hormone (LH) secretion, which prevents premature ovulation during fertility treatment.

Ganirelix is a synthetic decapeptide that competitively blocks gonadotropin-releasing hormone receptors on the pituitary. Blockade is immediate, so luteinising hormone release falls within hours rather than after the two-week down-regulation a GnRH agonist requires, and it reverses just as quickly when the drug is stopped.

It is used during ovarian stimulation for assisted reproduction, to prevent a premature LH surge releasing the eggs before they can be collected. Treatment starts on about day five or six of stimulation and continues daily until the trigger injection. The antagonist approach shortens the treatment cycle, reduces the amount of gonadotropin needed, and allows ovulation to be triggered with a GnRH agonist rather than hCG — which substantially reduces the risk of ovarian hyperstimulation syndrome, the most serious complication of IVF. Injection site reactions are the commonest adverse effect and are usually mild and short-lived.
