Menotropins, also called human menopausal gonadotropin, are purified gonadotropins extracted from the urine of postmenopausal women. Each vial contains both follicle-stimulating hormone activity, which drives follicle growth, and luteinising hormone activity, largely as hCG, which supports androgen production by the theca cells and thereby oestrogen synthesis.

It is used to stimulate the development of multiple follicles in assisted reproduction, and to induce ovulation in women who do not ovulate. The debate between urinary-derived menotropins and recombinant FSH has run for decades: recombinant products offer batch consistency and no LH activity, while menotropins provide the LH component that some protocols and some patients — particularly those with low endogenous LH — appear to benefit from. The serious risk is ovarian hyperstimulation syndrome, which requires monitoring of follicle counts and oestradiol, and multiple pregnancy.
