Spiramycin is a macrolide antibiotic. Like others in its class it binds the 50S ribosomal subunit and blocks bacterial protein synthesis, but it has a property the others lack and which defines its use: it concentrates heavily in the placenta, reaching levels several times those in maternal blood, while crossing into the fetal circulation only poorly.

That distribution makes it the standard treatment for toxoplasmosis acquired during pregnancy. Given to the mother, it reduces the risk of the parasite being transmitted across the placenta to the fetus — roughly by half — while exposing the fetus itself to very little drug. It does not treat established fetal infection, for which pyrimethamine and sulfadiazine are used instead once the second trimester is reached. It is also used for susceptible respiratory and oral infections. Gastrointestinal upset is the usual adverse effect; QT prolongation is a class caution.

Sourcing notes. Off patent, but registered in a limited set of countries — widely available in France and much of Europe, and not approved in the United States except through a named-patient route.
