Procaine Penicillin is a long-acting penicillin antibiotic used to treat bacterial infections, including syphilis, pneumonia, anthrax, and upper respiratory tract infections. It combines penicillin G with procaine to reduce injection pain and provide prolonged antibacterial activity.

Procaine penicillin is benzylpenicillin combined with procaine to form a poorly soluble salt, injected intramuscularly as a depot from which penicillin is released slowly over roughly twenty-four hours. That sustained release is the entire purpose: it allows once-daily dosing where aqueous benzylpenicillin would require administration every four to six hours.

It is used in syphilis, in streptococcal infection and in community-acquired pneumonia in settings where daily attendance is practical but intravenous access is not, and is supplied as 600,000 and 1,200,000 unit presentations.

Route is critical and errors are serious. Procaine penicillin must be given by deep intramuscular injection only. Inadvertent intravascular injection causes Hoigné syndrome — an acute, frightening reaction of anxiety, hallucinations and a sense of impending death, caused by procaine reaching the circulation — and can cause vascular occlusion and tissue necrosis. Labelling and staff instruction must state the route unambiguously.

Sourcing notes. Strength is expressed in international units rather than milligrams, and specifications occasionally mix the two. Distinguish clearly from benzathine penicillin, which is a longer-acting depot used for rheumatic fever prophylaxis; the two are not interchangeable. Manufacture requires sterile suspension capability. WHO classifies it under Access.
