Cefazolin is a beta-lactam antibiotic that works by inhibiting bacterial cell wall synthesis, leading to bacterial death. It is used to treat a variety of bacterial infections including skin and soft tissue infections, urinary tract infections, respiratory tract infections, bone and joint infections, endocarditis, biliary tract infections, and for surgical prophylaxis.

Cefazolin is a first-generation parenteral cephalosporin, and its dominant use is not treatment but prevention: it is the standard agent for surgical antibiotic prophylaxis across most clean and clean-contaminated procedures. Reliable activity against staphylococci and streptococci, a convenient half-life allowing a single preoperative dose, low cost and a good safety record together make it the default in surgical protocols worldwide.

It is supplied as powder for injection at 500 mg, 1 g and 2 g. In treatment it is used for skin and soft tissue infection, and remains a preferred agent for methicillin-susceptible Staphylococcus aureus bacteraemia, where it performs at least as well as antistaphylococcal penicillins with better tolerability.

Sourcing notes. Because surgical prophylaxis volumes are predictable and large, cefazolin is a high-volume hospital tender item and pricing is tightly competitive. WHO classifies it under Access. Manufacture requires sterile powder-filling approval at the specific site. Supply of cefazolin has been subject to periodic global shortage, driven by concentration of API manufacture in a small number of plants — a genuine consideration when committing to a long-term supply contract, and worth establishing how many API sources the finished-dose manufacturer qualifies.
