Product Description:

Cefoxitin is technically a cephamycin rather than a cephalosporin, and the difference is a methoxy group at the seven position that shields the beta-lactam ring from many beta-lactamase enzymes. It is grouped with the second-generation cephalosporins in practice. That structural protection gives it reliable activity against anaerobes, particularly the Bacteroides fragilis group, which most cephalosporins of its era could not touch.

Anaerobic cover is what defines its use. It is given intravenously for intra-abdominal and gynaecological infections, where gut flora mean aerobes and anaerobes together, for pelvic inflammatory disease, and for surgical prophylaxis in colorectal and gynaecological procedures. It also has a niche in rapidly growing mycobacterial infection. Hypersensitivity is the main adverse effect, with the usual caution about cross-reactivity in penicillin-allergic patients. The short half-life means frequent dosing, typically six-hourly.
