Daptomycin is a cyclic lipopeptide antibiotic used for the treatment of serious Gram-positive bacterial infections, including complicated skin and soft tissue infections, bloodstream infections (bacteremia), and right-sided infective endocarditis caused by Staphylococcus aureus. It works by disrupting bacterial cell membrane function, leading to rapid bacterial cell death.

Product Description:

Daptomycin is a cyclic lipopeptide with a mechanism unlike other Gram-positive agents: it inserts into the bacterial cell membrane in a calcium-dependent manner, causing rapid depolarisation and cell death without lysis. It is bactericidal against methicillin-resistant Staphylococcus aureus, vancomycin-resistant enterococci and other resistant Gram-positive organisms.

It is indicated for complicated skin and skin structure infection and for S. aureus bacteraemia including right-sided endocarditis, and is supplied as lyophilised powder at 350 and 500 mg.

One absolute limitation must be understood before it is supplied for respiratory use. Daptomycin is inactivated by pulmonary surfactant and is therefore ineffective in pneumonia. This is not a matter of reduced efficacy but of the drug not working at that site at all, and a specification listing daptomycin for a respiratory indication has been written in error.

Creatine kinase monitoring is required, since myopathy and rarely rhabdomyolysis occur.

Sourcing notes. Daptomycin is off patent and generic supply is well established from several manufacturers, so this is genuinely sourceable. Manufacture requires lyophilisation capability at the specific plant. WHO classifies daptomycin under Reserve, which restricts its use to confirmed resistant infection under stewardship control and affects tender eligibility.
