Imiquimod is a topical immune response modifier used for the treatment of various skin conditions, including actinic keratosis, superficial basal cell carcinoma, and external genital or perianal warts. It works by stimulating the local immune system, promoting the production of cytokines that help the body fight abnormal or virus-infected cells. Imiquimod is available in cream formulations and is applied directly to the affected skin area, offering a non-invasive treatment option for dermatological conditions.

Product Description:

Imiquimod does not kill abnormal cells directly. It is an immune response modifier that binds toll-like receptor 7 on dendritic cells and macrophages, triggering release of interferon-alpha, tumour necrosis factor and interleukins, which recruit a local cell-mediated immune response against virally infected or dysplastic cells. The inflammation it provokes is the treatment, not a side effect.

It is used for external genital and perianal warts, actinic keratoses, and superficial basal cell carcinoma where surgery is unsuitable, with different strengths and schedules for each. Patients must understand what to expect: marked redness, erosion, crusting and discomfort at the treated site are normal and correlate with response, but they can be severe, and rest periods are built into the regimens. Systemic flu-like symptoms occur. Treated skin should not be occluded, and sun exposure is avoided.

Sourcing notes. Off patent, with generics widely available. The strengths correspond to different indications and regimens and are not interchangeable, so a quotation must state which. Single-use sachets are standard for the genital wart and actinic keratosis regimens, and sachet count per pack is what determines whether a pack covers a full course.
