Retapamulin is a topical antibiotic used to treat impetigo caused by methicillin-susceptible Staphylococcus aureus or Streptococcus pyogenes. It works by inhibiting bacterial protein synthesis, preventing the growth and spread of bacteria in the affected skin area. The medication is typically applied as a 1% ointment to the affected area twice daily for 5 days. Product Description:

Retapamulin is a pleuromutilin, the first of that class developed for human topical use. It binds a site on the bacterial 50S ribosomal subunit that no other antibiotic class targets, blocking peptide bond formation. Because the binding site is unique, there is no meaningful cross-resistance with macrolides, fusidic acid, mupirocin or beta-lactams — which is its commercial argument, in a field where resistance to mupirocin and fusidic acid has grown with heavy topical use.

It is used for impetigo caused by Staphylococcus aureus and Streptococcus pyogenes, applied twice daily for five days to a limited area. It is active against methicillin-resistant strains in vitro, though its licensed indications generally cover methicillin-susceptible organisms. Local irritation at the application site is the main adverse effect and is uncommon. It is not for use on mucous membranes or for infections of larger areas, where oral therapy is appropriate.

Sourcing notes. Registration has narrowed since launch and the product has been withdrawn in some markets, so confirm the destination country rather than assuming availability. Small tube sizes reflect the short course and limited area. Where it is unavailable, mupirocin and fusidic acid are the locally registered alternatives, both with established resistance concerns.
