Mafenide is a topical antibacterial agent used primarily in the treatment of burn wounds to prevent and treat infections. It works by inhibiting bacterial growth on the skin surface. It is especially effective against a wide range of gram-positive and gram-negative bacteria.

Product Description:

Mafenide is a sulfonamide antibacterial formulated for burn wounds. Its defining property is penetration: unlike most topical antibacterials it diffuses through eschar, the thick devitalised tissue that covers a deep burn and that normally shields bacteria from surface agents. That makes it effective against established infection under eschar, particularly Pseudomonas aeruginosa and Clostridium species.

It is used for second and third degree burns, applied as a cream once or twice daily to the full wound surface, or as a solution soak on skin grafts and donor sites. Two adverse effects define its use. It inhibits carbonic anhydrase, so systemic absorption from a large burn causes metabolic acidosis with compensatory hyperventilation, which must be watched for and may require pausing treatment. Application is also painful, sometimes intensely so, which limits tolerability on extensive burns. Because it is a sulfonamide, hypersensitivity reactions occur.

Sourcing notes. A burns-unit product, not a general dermatology line, so buyers are hospitals and burn centres and demand is concentrated and inelastic. Cream and the powder for solution are separate registrations. It is frequently confused with silver sulfadiazine, a different drug used in the same setting, so quotations should be explicit about which is being offered.
