Halothane is a halogenated alkane and was the drug that made modern inhalational anaesthesia practical: non-flammable, potent and pleasant to breathe, it replaced ether and cyclopropane in the 1950s. It is a smooth induction agent with a sweet, non-irritant smell, which is why it remained the agent of choice for gas induction in children long after newer drugs appeared.

It is used for induction and maintenance of general anaesthesia. Two problems ended its dominance in wealthy countries. Halothane hepatitis is a rare but often fatal immune-mediated liver injury, more likely after repeated exposure, which led to guidance against re-administration within months. And halothane sensitises the myocardium to catecholamines, causing arrhythmias, while depressing cardiac output more than sevoflurane. It remains in use in many low and middle income countries, where it is inexpensive, works with simple draw-over vaporisers and needs no electricity — practical advantages that matter more than the alternatives' safety margin when neither is available.

Sourcing notes. Manufacture has contracted sharply as demand fell, and supply is now concentrated in a few producers serving LMIC markets. Bottles need light protection and contain thymol as a stabiliser, which accumulates in vaporisers and requires servicing.
