Prilocaine is a local anesthetic of the amide type, used in infiltration anesthesia, nerve blocks, and dental procedures. It is less cardiotoxic than lignocaine (lidocaine), making it useful in higher doses.

Product Description:

Prilocaine is an amide local anaesthetic with the lowest systemic toxicity of the common amides, because it is cleared rapidly and distributes widely. That safety margin makes it useful where larger volumes are needed, and it is the anaesthetic of choice for intravenous regional anaesthesia — the Bier's block used for forearm and hand surgery, where a large dose sits in an exsanguinated limb under tourniquet.

It is used for infiltration, nerve block, dental anaesthesia and intravenous regional anaesthesia, and a hyperbaric formulation is used for short spinal anaesthesia in day surgery. It is also half of the classic topical eutectic cream with lidocaine, used before cannulation in children. Its characteristic adverse effect is methaemoglobinaemia: a metabolite, o-toluidine, oxidises haemoglobin so it cannot carry oxygen. It is dose-related, and it matters most in infants under six months, in anaemia and in cardiorespiratory disease. Treatment is methylene blue.

Sourcing notes. Dental cartridges, plain injection, hyperbaric spinal solution and the topical cream are separate registrations with different buyers — dental supply is a distinct channel. Registration is stronger in Europe than in the US. Concentration and whether adrenaline or felypressin is included must be stated.
