Profound motor block is useful in abdominal surgery where muscle relaxation aids the surgeon, but it is a disadvantage almost everywhere else — particularly in obstetric epidurals and post-operative analgesia, where patients want pain relief without leg weakness, and in day surgery, where motor recovery determines discharge. Bupivacaine and later ropivacaine, which favour sensory over motor block, took the market. Cardiotoxicity at high plasma concentrations is a class concern with long-acting amides.
