Product Description:

Cidofovir is a nucleotide analogue of cytidine that already carries a phosphonate group, so it does not need the viral kinase that activates aciclovir and ganciclovir. Cellular enzymes complete the activation, which means it works against viruses resistant to those drugs through kinase mutations. Its very long intracellular half-life allows dosing every one or two weeks.

It is used for cytomegalovirus retinitis in AIDS, and off label for resistant adenovirus, BK virus and some poxvirus infections in transplant and immunocompromised patients. Its use is limited by one thing: severe, dose-dependent kidney damage. Every dose must be given with intravenous hydration and oral probenecid, which blocks the tubular transporter that concentrates cidofovir in kidney cells, and renal function and urine protein are checked before each dose. Neutropenia and ocular hypotony also occur. It is contraindicated with other nephrotoxic drugs.

Sourcing notes. The originator brand was discontinued and supply is now generic and narrow, so availability should be confirmed before any commitment. Probenecid must be available alongside it — a product from an entirely different supply chain that is itself subject to shortage.
