Tipranavir is an antiretroviral medication belonging to the protease inhibitor class. It is used in combination with ritonavir and other antiretroviral agents to treat HIV-1 infection in patients who have developed resistance to other HIV drugs. Tipranavir works by blocking the HIV protease enzyme, which prevents the virus from multiplying and lowers the amount of HIV in the blood.

Tipranavir is a non-peptidic protease inhibitor. HIV protease cuts the long polyprotein chains into functional proteins as new virus particles mature; blocking it produces immature, non-infectious virus. Tipranavir's non-peptidic structure lets it bind flexibly in the enzyme's active site, so it retains activity against protease resistant to other drugs in the class — its reason for use.

It is used with low-dose ritonavir, which boosts its levels, in combination therapy for HIV-1 in treatment-experienced adults with resistance to multiple protease inhibitors. Two boxed warnings define its safety profile: clinical hepatitis and hepatic decompensation, including fatal cases, so liver function is monitored closely and it is avoided in moderate to severe hepatic impairment; and intracranial haemorrhage, an effect unique among antiretrovirals, thought to relate to platelet aggregation, which means caution in patients with bleeding risk or on anticoagulants. It contains a sulfonamide moiety and interacts extensively through CYP3A4.

Sourcing notes. A salvage-therapy product with small, specialist demand, largely displaced by darunavir and by integrase inhibitors. Capsules and oral solution are separate registrations; the solution contains vitamin E in an amount that should be accounted for in supplementation. Confirm registration, as it has been withdrawn in some markets.
