Abacavir/Lamivudine is a fixed-dose antiretroviral combination used in the treatment of HIV-1 infection. It combines two nucleoside reverse transcriptase inhibitors (NRTIs) that work by inhibiting the reverse transcriptase enzyme, preventing viral replication and reducing viral load in patients.

Product Description:

This tablet combines two nucleoside reverse transcriptase inhibitors. Abacavir is a guanosine analogue and lamivudine a cytidine analogue; both are phosphorylated inside the cell and then incorporated into the growing viral DNA chain, where the missing hydroxyl group stops the chain being extended. Using two drugs from the same class but with different resistance patterns forms the backbone of HIV therapy, to which a third agent from another class is added.

It is used for HIV-1 infection in adults and adolescents, as one tablet once daily. The critical safety point is abacavir hypersensitivity: a systemic reaction with fever, rash, gastrointestinal and respiratory symptoms that can be fatal on rechallenge. It is strongly associated with the HLA-B*5701 allele, and screening for it before the first dose has made the reaction largely avoidable — testing is mandatory in guidelines. Lamivudine is also active against hepatitis B, so stopping it in a co-infected patient can cause a severe hepatitis flare.

Sourcing notes. Off patent, with generics widely available, and a major product in public HIV programmes where tender price decides awards. HLA-B*5701 testing capability in the destination market is a prerequisite for safe use.
