Carteolol ophthalmic is a topical beta-blocker used to lower intraocular pressure in patients with open-angle glaucoma or ocular hypertension. It works by decreasing the production of aqueous humor in the eye. Compared to other beta-blockers, Carteolol may cause fewer systemic side effects like bradycardia or bronchospasm.

Product Description:

Carteolol is a non-selective beta-blocker eye drop that lowers intraocular pressure by reducing aqueous humour production. What distinguishes it from timolol is intrinsic sympathomimetic activity: it partially stimulates beta receptors while blocking them. That partial agonism means less reduction in resting heart rate and, in studies, a smaller effect on the serum lipid profile than timolol, which is relevant in a patient group that is often elderly and cardiovascularly unwell.

It is used once or twice daily for open-angle glaucoma and ocular hypertension. The intrinsic activity does not remove the class risk: a large share of each drop drains through the nasolacrimal duct and is absorbed systemically, bypassing first-pass metabolism, so bronchospasm, bradycardia, heart block and masking of hypoglycaemia remain possible. It is contraindicated in asthma, severe COPD, sinus bradycardia and heart block. Pressing on the inner corner of the eye after instillation reduces systemic absorption substantially.

Sourcing notes. Registered in Europe, Japan and parts of Asia; it was discontinued in the US, so confirm the destination market. Long-acting gel-forming presentations exist in some markets as separate registrations. Off patent and inexpensive.
