Bromfenac is a nonsteroidal anti-inflammatory drug (NSAID) used in ophthalmology to reduce ocular inflammation and pain following cataract surgery. It works by inhibiting prostaglandin synthesis through COX-2 enzyme inhibition, thereby decreasing inflammation and discomfort in the eye.

Product Description:

Bromfenac is a topical non-steroidal anti-inflammatory drug. The bromine atom on the molecule makes it markedly more lipophilic than other ophthalmic NSAIDs, so it penetrates the cornea rapidly and reaches the retina and choroid in useful concentrations — which is why it can be dosed once or twice daily where older NSAIDs needed four times.

It is used for inflammation and pain after cataract surgery, and off label to prevent and treat cystoid macular oedema, where its posterior segment penetration matters. Its advantage over topical steroids in this setting is that it does not raise intraocular pressure. The risk that defines the class is corneal: topical NSAIDs can cause corneal epithelial breakdown, stromal thinning and, rarely, perforation, particularly in patients with dry eye, diabetes, rheumatoid arthritis, repeated ocular surgery or concurrent steroid use. Treatment beyond the licensed period is avoided for that reason.

Sourcing notes. Several concentrations exist, reflecting different dosing frequencies and preservative systems, and they are not interchangeable — a quotation must state which. Some presentations use an alternative preservative or a gel-forming vehicle. Off patent in most markets, with generics available, and surgical centres rather than pharmacies are the main buyers.
