Nepafenac is available as ophthalmic suspension in strengths 0.1% and 0.3%, supplied in sterile dropper bottles for ocular use. Short description: Nepafenac is a nonsteroidal anti-inflammatory drug used in the eye to reduce pain and inflammation associated with cataract surgery. It is a prodrug that converts to amfenac in ocular tissues, providing targeted anti-inflammatory effects. It is usually prescribed for short-term postoperative management.

Product Description:

Nepafenac is a prodrug NSAID. It is applied as the inactive amide, which penetrates the cornea readily because it is uncharged, and is then converted by intraocular hydrolases to amfenac, the active drug, inside the eye. That design concentrates activity where it is needed and reduces the corneal surface exposure that causes the epithelial problems associated with topical NSAIDs.

It is used to prevent and treat pain and inflammation after cataract surgery, and it has particular support for reducing the risk of cystoid macular oedema in diabetic patients undergoing cataract surgery, where the blood-retinal barrier is already compromised. The lower strength is given three times daily and the higher strength once daily; they are not interchangeable. Class cautions apply: corneal thinning and, rarely, perforation with prolonged use or in eyes with surface disease, delayed healing, and a small increase in bleeding tendency in operated tissue.

Sourcing notes. A suspension, so resuspendability and particle size are critical quality attributes and the bottle must be shaken. Two strengths with different dosing must be specified separately. Off patent in most markets. Surgical centres and ophthalmic chains are the buyers, with demand tracking cataract volume.
