Tetrahydrozoline, also known as tetryzoline, is a sympathomimetic imidazoline derivative primarily used as a topical decongestant in ophthalmic and nasal preparations. It is commonly found in over-the-counter eye drops and nasal sprays designed to provide temporary relief from redness, irritation, and congestion. The drug works by stimulating alpha-adrenergic receptors in the small blood vessels of the eyes or nasal mucosa, leading to vasoconstriction. This action reduces blood flow in the affected area, which helps decrease redness, swelling, and discomfort.

In ophthalmic formulations, tetrahydrozoline is often used to relieve eye redness caused by minor irritants such as dust, smoke, wind, chlorine exposure, or allergic reactions. In nasal sprays, it helps relieve nasal congestion associated with colds, sinus irritation, or seasonal allergies. The onset of action is usually rapid, providing noticeable relief within a short period after application.

Although generally well tolerated when used as directed, tetrahydrozoline is intended for short-term use. Prolonged or excessive use may lead to rebound redness in the eyes or rebound nasal congestion.
