This combination pairs xanomeline, a muscarinic agonist preferring M1 and M4 receptors, with trospium, a peripherally restricted muscarinic antagonist. Xanomeline reduces psychotic symptoms by acting on muscarinic receptors that regulate dopamine release in the striatum, without blocking dopamine D2 receptors — the mechanism behind every earlier antipsychotic. Alone, xanomeline caused intolerable nausea, vomiting and sweating through peripheral receptors. Trospium does not cross the blood-brain barrier, so it blocks those peripheral effects while leaving the central action intact.

It is approved for schizophrenia in adults. Because it does not block D2 receptors, it lacks the extrapyramidal symptoms, raised prolactin and weight gain that limit other antipsychotics. The main adverse effects are cholinergic and anticholinergic: nausea, vomiting, constipation, dyspepsia and increased heart rate. Urinary retention is a warning, and it is contraindicated in moderate to severe hepatic impairment and untreated narrow-angle glaucoma. Doses are taken on an empty stomach, twice daily.
