Eslicarbazepine acetate is an anticonvulsant (anti-epileptic) medication used in the treatment of partial-onset (focal) seizures with or without secondary generalization in adults and children. It is a prodrug that converts to eslicarbazepine, which works by stabilizing neuronal sodium channels, reducing repetitive firing of brain cells and helping to control seizures.

Eslicarbazepine acetate is a prodrug converted almost entirely to eslicarbazepine, the S-enantiomer of licarbazepine, which is oxcarbazepine's active metabolite. It stabilises the slow-inactivated state of voltage-gated sodium channels, preferentially affecting neurons firing repetitively during a seizure. Going straight to the single active enantiomer avoids some of the conversion variability of oxcarbazepine and allows once-daily dosing.

It is used for focal-onset seizures, as monotherapy or add-on, in adults and children. Hyponatraemia is the adverse effect that needs watching, as with oxcarbazepine, particularly in older patients and those on diuretics. Dizziness, diplopia and ataxia are common during titration. It shares the class risk of severe cutaneous reactions, and screening for HLA-B*1502 before starting is advised in patients of Han Chinese, Thai and some other Southeast Asian ancestry. It induces CYP3A4 and can lower hormonal contraceptive levels.

Sourcing notes. Generic entry has begun but its timing differs between markets, so confirm patent and exclusivity position country by country before quoting. Tablets are large, which matters for adherence, and the oral suspension is a separate registration for paediatric use. Where it is registered, it is often positioned as a once-daily switch for patients on oxcarbazepine; in markets without it, oxcarbazepine is the realistic alternative.
