Vigabatrin is an anti-epileptic (anticonvulsant) medication used for the treatment of refractory complex partial seizures in adults and children, as well as infantile spasms in infants. It works by irreversibly inhibiting GABA-transaminase, an enzyme that breaks down GABA, leading to increased levels of the inhibitory neurotransmitter GABA in the brain. This helps stabilize neuronal activity and control seizures.

Vigabatrin is a structural analogue of GABA that irreversibly inhibits GABA transaminase, the enzyme that breaks down GABA. Because the inhibition is irreversible, its effect lasts until the enzyme is resynthesised, well beyond the drug's plasma half-life. Brain GABA levels rise several-fold. It is first-line treatment for infantile spasms, particularly in tuberous sclerosis complex, and is used as add-on therapy for refractory focal seizures.

Its use is defined by its main risk: permanent bilateral concentric visual field loss, which occurs in a large proportion of long-term users, cannot be reversed, and may not be noticed by the patient. Ophthalmological monitoring is required, and in the US the drug is distributed under a restricted programme. Treatment is reassessed if there is no clear benefit within weeks. It can worsen absence and myoclonic seizures. MRI signal changes have been seen in infants.

Sourcing notes. Generic tablets and powder for oral solution are available in the US and Europe, and a ready-to-use oral solution has been approved in the US. Infant dosing uses sachets or solution, not tablets, so the paediatric presentation drives demand. In the US every supplier must participate in the vision-monitoring programme. Registration elsewhere is often restricted to specialist centres.
