Perampanel is an antiepileptic drug used to treat partial-onset seizures and primary generalized tonic-clonic seizures in patients with epilepsy. It acts as a selective, non-competitive antagonist of the AMPA receptor on postsynaptic neurons, reducing excessive neuronal excitation that can lead to seizures.

Perampanel is a selective, non-competitive antagonist of the AMPA-type glutamate receptor — the first antiepileptic acting on that target. AMPA receptors mediate the fast excitatory transmission through which seizures spread, so blocking them reduces seizure propagation. It is used for focal-onset seizures and, as add-on, for primary generalised tonic-clonic seizures.

A half-life of about a hundred hours permits a single bedtime dose, which also takes advantage of its sedative effect. Enzyme-inducing antiepileptics such as carbamazepine, oxcarbazepine and phenytoin shorten the half-life markedly and require higher doses. The most important adverse effects are psychiatric: irritability, aggression, hostility and, rarely, homicidal ideation, which carry a boxed warning in the US. Dizziness, somnolence and falls are common, particularly at higher doses and in older patients. Titration is slow, in 2 mg steps no more often than weekly.

Sourcing notes. Perampanel is Schedule III in the US because of its abuse potential at high doses, which brings registration and handling requirements there; most other markets do not schedule it. Generic entry has begun but varies by country, so confirm exclusivity locally. The tablets come in six strengths to support slow titration, and the oral suspension serves paediatric use.
