Dantrolene is a skeletal muscle relaxant used to treat muscle spasticity resulting from conditions like multiple sclerosis, cerebral palsy, or spinal cord injury. It’s also a life-saving drug used in the emergency treatment of malignant hyperthermia, a rare but severe reaction to anesthesia. It works by reducing calcium release from muscle cells, leading to muscle relaxation.

Dantrolene is the only specific treatment for malignant hyperthermia. It inhibits the ryanodine receptor RYR1 on the sarcoplasmic reticulum of skeletal muscle, stopping the uncontrolled calcium release that volatile anaesthetics and succinylcholine trigger in susceptible patients. Without it, mortality from a malignant hyperthermia crisis was historically in the region of seventy to eighty percent.

Orally it is used for chronic spasticity after spinal cord injury, stroke, cerebral palsy and multiple sclerosis. Because it acts on the muscle rather than the spinal cord it causes less sedation than baclofen or tizanidine, but the weakness it produces can offset the gain in function. Oral use carries a boxed warning for hepatotoxicity, with risk rising with dose and duration, so liver function is monitored.

Sourcing notes. The intravenous product is the critical one, and its formulation is the main constraint. Conventional 20 mg vials must each be reconstituted in 60 mL of water, and a single crisis can need more than thirty vials — hospitals running volatile anaesthesia are expected to keep a full stock on hand. A concentrated formulation reconstitutes 250 mg in 5 mL and is far faster to prepare, but it is a separate registration and priced accordingly. Stocking is usually mandated by professional anaesthesia guidance, so demand is steady even though use is rare.
