Droxidopa is a synthetic amino acid analog and a pro-drug of norepinephrine which is converted to norepinephrine by dopa-decarboxylase. It is indicated for adult patients with symptomatic neurogenic orthostatic hypotension (NOH) caused by primary autonomic failure (such as Parkinson’s disease, Multiple System Atrophy, or pure autonomic failure), dopamine β-hydroxylase deficiency, or non-diabetic autonomic neuropathy. The drug helps prevent dizziness, lightheadedness or fainting upon standing by increasing norepinephrine levels and improving vasoconstriction. Key cautions include risk of supine hypertension

Product Description:

Droxidopa is a synthetic amino acid prodrug that dopa decarboxylase converts directly into noradrenaline, bypassing dopamine beta-hydroxylase. It is used for symptomatic neurogenic orthostatic hypotension — the drop in blood pressure on standing seen in Parkinson's disease, multiple system atrophy, pure autonomic failure and dopamine beta-hydroxylase deficiency — where the sympathetic nervous system can no longer release enough noradrenaline to maintain upright blood pressure.

The central management problem is that the same patients who faint when standing develop hypertension lying down. Droxidopa carries a boxed warning for supine hypertension; the last dose is taken at least three hours before bedtime and patients sleep with the head raised. Durable efficacy beyond two weeks was not established in the pivotal trials, which is why US approval was accelerated, and response is reassessed periodically. Carbidopa, taken by most Parkinson's patients, inhibits the conversion.

Sourcing notes. Registered for this indication mainly in the US and Japan, where it originated; much of Europe does not register it at all, and midodrine is the usual alternative. Generic capsules exist in the US. Buyers outside those markets should expect named-patient or special-import routes. Capsule manufacture is conventional, and demand is small and specialist, driven by movement-disorder and autonomic clinics.
