Trimetazidine is a metabolic agent used in cardiology to improve heart function. It works by optimizing myocardial energy metabolism, helping the heart use glucose more efficiently under conditions of low oxygen. Product Description:

Trimetazidine is an anti-anginal drug that works on cardiac metabolism rather than on blood flow or heart rate. It partially inhibits the enzyme 3-ketoacyl-CoA thiolase, shifting the heart away from fatty acid oxidation towards glucose oxidation. Glucose oxidation uses less oxygen per unit of energy produced, so the heart copes better when blood supply is limited. It does not lower blood pressure or heart rate, which is why it can be added to drugs that do.

Its place is defined by a 2012 European review. Because of reports of parkinsonian symptoms, tremor, restless legs and gait disorders, it was restricted to second-line add-on treatment for stable angina in patients inadequately controlled or intolerant of first-line therapy, and it was contraindicated in Parkinson's disease and movement disorders. Earlier uses in vertigo and eye disorders were removed. Symptoms usually resolve within months of stopping. The dose is reduced in moderate renal impairment and it is contraindicated in severe impairment.

Sourcing notes. Registered in Europe, Asia, Russia and Latin America, but never in the US. Generic modified-release tablets are widely available. Labelling must carry the restricted indication and the movement-disorder contraindication.
