Product Description:

Vericiguat is a soluble guanylate cyclase stimulator. In heart failure, oxidative stress reduces nitric oxide availability and leaves the enzyme less responsive, so production of cyclic GMP falls, and with it the signalling that relaxes blood vessels and protects the heart and vessels from remodelling. Vericiguat stimulates the enzyme directly and also makes it more sensitive to what nitric oxide remains, restoring that pathway.

It is used in symptomatic chronic heart failure with reduced ejection fraction in patients who have recently been hospitalised or needed intravenous diuretics — a high-risk group. In its pivotal trial it reduced cardiovascular death or heart failure hospitalisation, with the benefit driven mainly by fewer hospitalisations. It is added to standard therapy rather than replacing any of it, and is titrated over weeks. Hypotension and anaemia are the main adverse effects. It must not be combined with PDE5 inhibitors or other soluble guanylate cyclase stimulators, and it is contraindicated in pregnancy.
