Tolfenamic acid is a fenamate NSAID. Like other fenamates it inhibits COX enzymes and also antagonises prostaglandin receptors directly, and it has some effect on the release and action of the peptides involved in migraine. That combination underlies its main clinical use.

It is used for acute migraine attacks, where trials support it as an effective alternative to triptans for some patients, and for general mild to moderate pain and musculoskeletal conditions. For migraine it is taken at the onset of headache, with a repeat dose permitted after one to two hours. Adverse effects are those of NSAIDs — dyspepsia, nausea, gastrointestinal bleeding with longer use, fluid retention and renal effects — plus a distinctive one: transient dysuria and urinary tract symptoms, reported more often with tolfenamic acid than with other NSAIDs. It should be avoided in peptic ulceration, significant renal impairment and the third trimester of pregnancy.
