Thiopental is an ultra-short-acting barbiturate that was, for decades, the standard induction agent for general anaesthesia. It enhances GABA-A receptor activity, producing unconsciousness within seconds of injection; the patient wakes not because the drug is eliminated but because it redistributes from brain to muscle and fat, which is also why repeated doses accumulate and prolong recovery.

Beyond induction, it retains specific roles: it lowers cerebral metabolic rate and intracranial pressure, so it is used in barbiturate coma for refractory raised intracranial pressure and refractory status epilepticus, where propofol is an alternative but not always sufficient. Propofol replaced it for routine induction because recovery is cleaner. It causes dose-dependent respiratory and cardiovascular depression, and it is contraindicated in acute porphyria, where it can precipitate a crisis.
