Macrogol is an osmotic laxative used to treat constipation and faecal impaction, and in higher doses for bowel cleansing before diagnostic procedures. It works by retaining water in the intestine, softening stools, and increasing bowel movement frequency.

Macrogol 3350, also called polyethylene glycol 3350, is an osmotic laxative. It is a large, inert polymer that is not absorbed or metabolised by gut bacteria, and it holds water in the bowel by hydrogen bonding. The retained water softens stool and increases its volume, which stimulates movement. Because it is not fermented, it causes less bloating and gas than lactulose.

At standard doses it treats chronic constipation and faecal impaction, and it is the first-line osmotic laxative in most guidelines, including for children. Effect usually takes one to three days. At much higher volumes, combined with electrolytes, it is used for bowel cleansing before colonoscopy. Formulations with electrolytes (such as Movicol) and without them (such as MiraLAX) are different products: electrolytes prevent net fluid and salt shifts during higher or prolonged use, which matters in older patients and renal or cardiac disease.

Sourcing notes. A high-volume, low-cost product. The critical quality control is ethylene glycol and diethylene glycol impurity limits in the polymer, since contamination with these has caused fatal poisonings worldwide. Buyers should require the pharmacopoeial grade with these limits tested batch by batch. Sachet integrity and moisture protection are packaging points.
