Fondaparinux is a synthetic anticoagulant that inhibits factor Xa, an essential enzyme in the blood clotting process. It is used to prevent and treat deep vein thrombosis (DVT) and pulmonary embolism (PE) and to prevent blood clots after major orthopedic or abdominal surgery.

Product Description:

Fondaparinux is a synthetic pentasaccharide — a chemical copy of the five-sugar sequence in heparin that binds antithrombin. Binding changes antithrombin's shape so that it inactivates factor Xa several hundred times faster. Because the molecule is too short to bridge antithrombin to thrombin, it inhibits factor Xa alone, with no effect on thrombin, and it does not bind platelet factor 4.

It is used to prevent venous thromboembolism after major orthopaedic and abdominal surgery, to treat deep vein thrombosis and pulmonary embolism, and in acute coronary syndromes. Its particular value is that, not binding platelet factor 4, it does not cause heparin-induced thrombocytopenia and is used to treat it. It is given once daily subcutaneously with no routine monitoring. It is cleared entirely by the kidneys, so it accumulates and is contraindicated in severe renal impairment, and unlike heparin it has no effective antidote — protamine does not reverse it.

Sourcing notes. Off patent, with generics in the US and Europe, though synthesis is long and complex for a small molecule, which keeps the manufacturer base narrow. Prefilled syringes with safety devices are standard, and the three strengths serve prophylaxis and treatment at different body weights.
