Product Description:

Triheptanoin is a synthetic triglyceride carrying three seven-carbon fatty acid chains. Odd-chain fatty acids are broken down into both acetyl-CoA and propionyl-CoA, and propionyl-CoA is converted to succinyl-CoA, which enters the Krebs cycle directly. That second, anaplerotic route is the point: in long-chain fatty acid oxidation disorders, the body cannot break down ordinary long-chain fats for energy, and the cycle runs short of intermediates. Triheptanoin refills it.

It is used as a source of calories and fatty acids in children and adults with confirmed long-chain fatty acid oxidation disorders, replacing a set proportion of daily calorie intake and taken with meals as part of a managed dietary regimen. In these conditions, which cause muscle breakdown, low blood sugar and cardiomyopathy during fasting or illness, it reduces hospitalisation in clinical experience. Abdominal pain, diarrhoea and vomiting are common, especially at first, and are managed by dividing doses.

Sourcing notes. Originator orphan product, on patent, approved in the US; availability elsewhere is generally through named-patient import.
