Glutamine is the most abundant amino acid in blood and the main fuel for rapidly dividing cells, including the lining of the gut and immune cells. It is classed as conditionally essential: the body normally makes enough, but during severe illness, burns, trauma or major surgery, demand outstrips supply and blood levels fall. Supplementation aims to replace that shortfall, supporting the intestinal barrier and nitrogen balance.

A 15 g sachet is a typical nutritional dose, mixed into water or a feed. The evidence is mixed and depends heavily on the setting. Enteral glutamine has been used in burns and in preventing chemotherapy-related mucositis with some supportive data. In critically ill patients on organ support, however, a large randomised trial found high-dose supplementation increased mortality, so it is not recommended in that group, particularly with renal or hepatic failure. In liver failure it adds a nitrogen load that can worsen encephalopathy.

Sourcing notes. Classification depends on the claim: a plain amino acid supplement, a food for special medical purposes, or medical nutrition, each with different labelling rules. L-glutamine is hygroscopic and degrades in solution, so sachets need moisture protection and use soon after mixing.
