Enteral nutrition

Enteral nutrition means feeding through the gastrointestinal tract when a patient cannot eat enough by mouth — by sip feed, nasogastric tube, or gastrostomy. It is preferred over intravenous feeding whenever the gut works at all, because using the gut preserves the intestinal barrier, maintains gut-associated immune tissue and reduces the risk of bacterial translocation and line infection.

A standard general-purpose formula provides protein, carbohydrate, fat, vitamins, minerals and trace elements in balanced proportions, typically at around one kilocalorie per millilitre when reconstituted, for patients with normal digestion and absorption and no specific organ requirement. Specialised formulas exist for renal failure, hepatic disease, respiratory failure and diabetes, and for impaired absorption, where peptide-based or elemental feeds are used. Refeeding syndrome is the serious risk when nutrition is restarted in a severely malnourished patient, and requires cautious initiation with electrolyte monitoring.

Sourcing notes. Enteral feeds are specified by numbers, not names: energy density, protein grams per 100 mL, osmolality, fibre content, and whether the product is intact-protein, peptide-based or elemental. A full nutritional composition sheet is needed before this can be matched to an enquiry. Powder and ready-to-hang liquid have very different shipping economics and in-use handling.
