Sargramostim is a recombinant human granulocyte-macrophage colony-stimulating factor (GM-CSF) used to stimulate the production of white blood cells. It is prescribed to patients undergoing bone marrow transplantation, chemotherapy, or with severe neutropenia to reduce infection risk. Administered via subcutaneous or intravenous injection, sargramostim helps accelerate bone marrow recovery and improve immune function.

Product Description:

Sargramostim is recombinant human granulocyte-macrophage colony-stimulating factor, produced in yeast. It stimulates bone marrow progenitor cells to produce neutrophils, monocytes and macrophages, and activates mature cells. It differs from filgrastim, which stimulates the neutrophil line alone; the broader effect on monocytes and macrophages is both its distinguishing feature and the reason it tends to cause more systemic symptoms.

It is used to shorten neutrophil recovery after induction chemotherapy in acute myeloid leukaemia, to mobilise stem cells and support engraftment after transplantation, and in graft failure. It also has an approval for acute radiation syndrome after significant exposure, which is why it is held in national emergency stockpiles. Fever, bone pain, rash and injection-site reactions are common, and a first-dose reaction with flushing, hypotension and breathlessness can occur.

Sourcing notes. A single-source originator biologic with no biosimilar; the yeast-derived molecule differs from bacterially produced GM-CSF products used elsewhere, so these are not interchangeable. Stockpile procurement is a distinct channel from hospital supply and is usually government contracted. Cold chain, with lyophilised and solution presentations differing in handling.
