Epinephrine injection is used for emergency treatment of severe allergic reactions (including anaphylaxis) to insect bites or stings, medicines, foods, or other substances. It is also used to treat anaphylaxis caused by unknown substances or triggered by exercise

Product Description:

Epinephrine (adrenaline) is the first-line treatment for anaphylaxis, and no other agent substitutes for it. It acts on both alpha and beta adrenergic receptors: alpha-mediated vasoconstriction reverses hypotension and mucosal oedema, while beta effects produce bronchodilation and increased cardiac output. Antihistamines and corticosteroids are adjuncts, and delay in administering epinephrine is the principal factor in fatal outcomes.

Presentations divide into two commercially distinct propositions. Ampoules and vials at 1 mg per mL supply hospitals and clinics. Pre-filled auto-injectors at fixed 0.15, 0.3 and 0.5 mg doses supply community and patient-held use, and are a device-plus-drug combination product with an entirely different regulatory and manufacturing pathway.

Handling constraints that determine whether a shipment survives. Epinephrine is oxidised by light and degrades at elevated temperature, discolouring to pink or brown as it does so. Product must ship protected from light and within its specified temperature range, and consignments arriving through hot-climate ports without adequate protection are a recurring source of rejection at receipt.

Sourcing notes. Establish at the outset whether the requirement is the ampoule or the auto-injector, since they share almost nothing beyond the molecule. Auto-injectors carry device registration requirements and far longer approval timelines. Epinephrine appears on the WHO Model List of Essential Medicines and features in most national emergency procurement programmes.
