Diphenhydramine injection is used to treat allergic reactions, especially for people who are unable to take diphenhydramine by mouth. It is used also to treat motion sickness.

Diphenhydramine hydrochloride is the archetypal ethanolamine first-generation antihistamine, and the injectable presentation exists for situations where the oral route is unavailable or too slow — acute allergic reactions, treatment of drug-induced dystonic reactions, and use as an adjunct alongside epinephrine in anaphylaxis, where it is supportive rather than primary treatment.

Its pharmacology extends well beyond H1 antagonism: it possesses significant anticholinergic, antiemetic and sedative properties, which accounts for its use in motion sickness and in acute extrapyramidal reactions. The standard parenteral strength is 50 mg per mL, though 10 mg per mL presentations are supplied in several markets for paediatric and titrated dosing.

Diphenhydramine appears on the WHO Model List of Essential Medicines and is stocked in essentially every hospital formulary worldwide.

Sourcing notes. Volumes in institutional and government tenders are substantial and pricing is highly competitive. Manufacture requires sterile small-volume parenteral capability at the specific plant — a company-level claim is not sufficient, and the approval must cover ampoules or vials as quoted. Because this is a hospital-stocked emergency product, remaining shelf life at delivery is frequently a tender disqualifier; agree the minimum acceptable shelf life at receipt in writing, not the shelf life at manufacture.
